Purpose: To explain to families that participation in evaluation activities is optional and that they can participate in HIPPY even if they don’t participate in the evaluation. Also, if they sign the consent form, they are still allowed to refuse to complete a survey/assessment at any time during the program year.
Consent forms will be delivered by a member of the A+ Team via Zoom to each family.
Other: Consent from every parent must be secured every year, and sent in to UNT/KFAC as applies.
FAMILY APPLICATION / UPDATE / EXIT
Purpose: To gather demographic data. While not an assessment per se, there are pre-post data points that are gathered with this form, such as amount of time spent reading, guardian education, guardian smoking, etc.
Training: None
Other: The same form is used for Application (pre) and Update/Exit (post).
BRACKEN SCHOOL READINESS ASSESSMENT - BSRA
Purpose:
- To assess the school readiness of children by measuring their knowledge of colors, letters, numbers, shapes, and sizes.
- To identify children who are "ready for school" and no longer need the intensity of weekly home visits in Age 5/Yr three of the program.
- To assess the school readiness of children being recruited, to ensure we serve children who need the program the most.

PARENT KNOWLEDGE OF ASD
Purpose: To measure knowledge of Autism Spectrum Disorder among parents of children with Autism

PARENTING CHECK IN - PCI
Purpose: To measure parental stress and other factors.
Training: UT provides

- Do NOT send UT the PCI’s collected on A+ families, UNLESS they are MIECHV families too.

GARS 3
Purpose: To assess where children in the A+HIPPY Program are in the Autism Spectrum
Training:
- This guide shows how to complete the GARS form
- This 7 minute Webinar reviews the purpose and method used to complete the assessment
- The GARS Level Scoring Guide (Spanish) helps home visitors navigate a conversation with parents about what each level means and how they correlate with their child’s individual GARS scores
Other: Scoring must be done by KFAC staff. This means that the assessment must be done and scanned to KFAC as soon as possible so the curriculum plan can be tailored to the need of each child.
PARENT INVOLVEMENT INTERVIEW – PII
Purpose: To assess the level and type of parental involvement in the child's education
Training: Powerpoint with audio

PROTECTIVE FACTORS SURVEY/HIPPY PARENT SURVEY (PFS/HPS)
Purpose: Assess the level and type of protective factors and support parents have.

PICCOLO and HIPPY Parent-Child Observation / A+ Version
Purpose: To assess parent child interaction (PICCOLO) and as it relates to understanding of the curriculum techniques (HPCO/A+)

- Conduct observation with each child if there are siblings served in the program
- Conduct POST for all parents exiting early
- No VT entry for PC Observation Form data. Set up tracking of completion of assessment using Set up>User Fields>PCI
ASQ-3 and ASQ-SE2

SOCIAL COMMUNICATION CHECKLIST
Purpose: To identify children who may need further professional assessments on possible developmental delays
Training:
- Refer to manuals that came with materials ordered.
- Use ASQ Referral Process for follow up and recording requirements.
- Tool to brainstorm referral ideas (ASQ Referral Ideas)
- Do NOT purchase the newer version of the ASQ3/SE as it does not match VT data entry.
- Referrals required for children who score at risk.
Purpose: To inform families of the child's current skills and assist in goal-setting.
Training: Overview was covered during in-person training, if any questions arise, address during 1:1 meetings.

Purpose: To screen guardians to identify risk factors that may be associated with depression and make referrals as needed.

DOMESTIC VIOLENCE / PARTNER RELATIONSHIP SCREENING TOOL
Purpose: A screening tool to help parents identify possible intimate partner violence and make referrals as needed.

TIPS:
- Review all training powerpoints prior to presenting to the home visitors. You may download and add slides without affecting the audio portion
- Download the powerpoint first (and enable editing if that pops us), audio may not play if using the version online
- Right click on mouse to pause audio and right click to resume audio
- Depending on audience understanding, you may want to pause at each slide and read through the bullet points. The audio enhances the information on the slide
- Pause for questions, and for pair and share activities
- Write questions down as you go and any unanswered questions can be sent to TX HIPPY for clarification
